Millennium Fellowship Project: Chronicare
Chronicare is an innovative healthcare solution designed to address the challenges faced by patients with chronic illnesses and the healthcare providers who care for them. The project aims to develop a centralized, secure, and accessible system for managing and sharing patient medical histories across multiple healthcare facilities. By doing so, Chronicare seeks to improve continuity of care, facilitate early intervention, and enhance treatment outcomes.
Chronic illnesses often require long-term, coordinated care that is hindered by fragmented medical records scattered across different facilities. This lack of a cohesive system can lead to delayed diagnoses, redundant tests, and suboptimal treatment plans. Chronicare solves this by offering an integrated platform where authorized healthcare providers can quickly retrieve a patient’s medical history, enabling informed decision-making and personalized care.
About the Millennium Fellow
Ivy Damaris Butichi is a passionate pharmacy student with a strong focus on research and drug development. Born in Mombasa, Kenya, and now residing in Nairobi, she is dedicated to improving healthcare accessibility for all. Through her innovative project Chronicare, Ivy aims to transform the healthcare system by simplifying the transfer and management of patient records, which can enhance treatment outcomes. Her desire to give back to the community led her to establish the ROHO Foundation Kenya, a platform for making a positive social impact. Ivy believes in being the change she wants to see in the world and actively pursues opportunities to realize that vision.
